Attendance is required. If a student is confronting an extenuating circumstance, they may request an excused absence. Requests will be reviewed in alignment with the college’s already established criteria for granting excused absences. Please note that the submission of the excused absence request form is required.

Tickets (Harlem Campus)
Please note that the Apollo Theater has been reserved for faculty and students. Students will be provided with six tickets each for friends and family. The White Coat ceremony tickets distribution will be announced at a later date.
ALL GUESTS ARE REQUIRED TO HAVE TICKETS TO ENTER THE THEATER. THERE ARE ABSOLUTELY NO EXCEPTIONS FOR LOST OR MISPLACED TICKETS.
Students seeking additional tickets should communicate with the Student Government President and Vice President. They will organize a distribution process and schedule.
The person who robes the student during the ceremony may be a family member or faculty with a doctoral degree in the health professions (i.e., DO, MD, Pharm.D, DDS, DPM, DPT, PhD in biological sciences, etc.) Please note there can only be one person per student for robing.
If you would like a designated person to robe you at the white coat ceremony, please complete the Harlem DO Class 2024 White Coat Robing Contact Information form by, December 1, 2021.
Students (only) are now invited to obtain the following passes in preparation for the ceremony. Family will be addressed at another time. As you complete each step, please print and save a copy of your submission to the Apollo Theater. Retain this copy and bring it with you on the date of the event, along with any other articulated requirements.

- GREEN PASS (health questionnaire| verified safe to enter venue): to be announced at a later date.
- Covid19 PCR test result: 48 hours prior to the event. The submission of a rapid test result is approved for this event.
Proof of Vaccination: Every person entering the venue must show a completed vaccination record (dated at least 14 days before the date of entry). Proof of vaccination can be displayed on your smartphone; however, a physical copy is preferred. The Apollo encourages the use of the New York State Excelsior Pass (http://epass.ny.gov), which provides a secure digital proof of vaccination. Downloading the app in advance and registering on the app prior to your visit will create a smoother entry process into the venue.
Bring a Mask: All attendees, artists, staff, crews and visitors are required to wear masks when not actively eating or drinking. Attendees voluntarily assume all risks of exposure to communicable disease & viruses and hereby waives all potential claims relating to such risks. If you are unable to furnish proof of vaccination with a valid picture ID at the time of your visit, entry will be denied.
COVID Test Required: In an effort to provide a safer environment for all, in addition to being fully vaccinated, all onsite personnel/visitors/artists must provide a negative COVID-19 test that is dated no sooner than 48 hours in advance of date of entry to the venue. Cost related to testing are the sole expense of the visitor, renter, presenter or producer; including the cost associated with venue personnel.
Health Questionnaire & Waiver
Fill out the online waiver, located in the link below, prior to your visit. The waiver form should not be completed more than one day prior to the visit. An email receipt will be sent to you upon completion. Retain a copy of your online registration in case you are asked to present it upon arrival to the theater.
DAY OF VISIT: We ask that you wear the appropriate personal protection equipment (PPE, masks) and must be worn at all times during your visit of Apollo's Historic Theater. Additional PPE, masks, will be available on-site. Hand sanitizer stations are available on each level of the theater.
