Instructions:
1st Rotation:
On the first day of your rotation please report to the Residency Education Office, 2nd floor, Heart Tower, room 257 at 9AM.

Please call Dr. Widmann at (718) 226-1683 Nancy Hogle at (718) 226-5912 if you can not find the office.

It is required that you complete all the following BEFORE interacting with staff and patients:

  1. Demographic Sheet
  2. Corporate Compliance Training and Code of Ethical Conduct Certification form
  3. Information Services Password Request Form
  4. Lending Library Form

After the above are completed, you will be given a letter to take to Security to get an Identification Badge.

You will be able to use you ID for entry and exit from the employee/staff parking areas, for which there is no fee.

Daily during your rotation, you must:

  1. You may turn in toll receipts or EZ Pass statements for reimbursement monthly.
  2. Appropriate attire is expected—clean white coat covering scrubs, shirt and ties otherwise. Women should be appropriately modestly dressed.
  3. Facial hair is to be clean shaven each AM; if a beard is preferred, it should be neatly trimmed.

2nd and/or 3rd Rotation(s):
On subsequent rotations students must follow the above instructions for 1st Rotation.

On the last day of your SURGERY rotation, you must:

  1. Turn in your Identification badge to the Residency Education Office 2nd floor, Heart Tower, room 257.
  2. Complete all evaluations of rotations (paper forms to be handed in for each rotation).
  3. Return Lawrence Essential of General Surgery Book.
  4. Return your beeper/pager.

Failure to comply with the check-out policy will result in delay the distribution of the final reimbursement check.

MEDICAL STUDENT PROCEDURE – 1st Day

Arrive by 9:00 AM
Park in the visitor parking in the east garage for the first day –- Students will receive security allowances upon arrival.
Meet at Summit Pediatrics in the MAC I building, Suite 10.
Students should be wearing professional attire, as well as a lab coat.
You will receive orientation material on your first day

MEDICAL STUDENT PROCEDURE – 1st Day

  • Be at the hospital (and ready to report) by 8:00 am.
  • Once you are on Williamson Street, proceed to the end of road where you will see the hospital on the right and the parking garage on the left. Park in that parking garage and take a white parking ticket at the entrance and park on the 4th or 5th level.
  • Bring the ticket with you to be validated for free parking.
  • Enter the hospital’s main entrance. Ask at the front desk for Charlotte Leonard on 7 North.
  • The Physician Services Office is next to the elevator on 7 North.
  • Come in the office and ask for Charlotte.
  • Attire: Business dress and white lab jacket - NO denim or sandals please.
  • A $10 deposit is required for the ID and parking sticker. This deposit will be returned at the end of the rotation when the cards are returned. The license plate number of your car is also required.
